In Step 3 of the process outlined on our website, or when the Human Rights & Equity Advisor is contacted, you are required to complete the Human Rights Concern/Complaint Intake Form. The submission of the form helps us to assess your concern/complaint in a timely matter, by gathering as much information as possible. This form is intended for students, parents/guardians/caregivers, and/or community members with concerns or complaints about code-based matters.
Please note that with many concerns or complaints, your child’s classroom educator is the first point of contact, followed by your school administration (Principal/Vice-Principal), then your Family of Schools Superintendent of Education, with any outstanding matters being directed to the Human Rights and Equity Advisor, followed by the Director of Education.
Durham Catholic District School Board (DCDSB) will keep all reports and the details contained in them confidential. Confidentiality means that, should we need to share information, we will do so discreetly and only share as much information as is necessary, and only with those individuals who need to be aware in order to address, review and/or resolve the concern. The Human Rights and Equity Advisor (HREA) may also need to share information with the responsible DCDSB leader(s) to address or resolve concerns that may have an ongoing or wider impact. The HREA decision to share information is guided by the principles of the DCDSB’s responsibility to maintain school-based and workplace environments free of harassment and discrimination. Any time the HREA shares information in this way, it is done discreetly and with a commitment to supporting the confidentiality of the individual(s) who made the report.
